CMDHB Service Integration

In 2013, Counties Manukau District Health Board (CMDHB) formally engaged National Hauora Coalition (NHC) to implement a service integration strategy with a group of Māori Health providers through an outcomes based contracting methodology. The aim of the strategy is to improve outcomes of service populations by using the NHC outcomes framework to align, design and implement:

1. Approaches that work for whānau

2. Transparent accountability for performance

3. A mechanism to develop local solutions.

The providers engaged in this strategy are as follows:

  • Health through the Marae
  • Huakina Trust
  • Manurewa Marae
  • Papakura Marae
  • Raukura
  • Port Waikato Community Health & Social Services Trust
  • Te Hononga
  • Te Kaha ote Ranagtahi
  • Turuki Healthcare

Over time, NHC envisages that the Service Integration Strategy will involve other providers and multiple funding agencies to further enhance outcomes for whānau.
